Please print out these 3 forms and mail the last 2 to me (address below).
These classes are on a first-come-first-serve basis. Since I have a limited classroom space available for each class I have found this is the only fair way of doing this.
If you would like to get on the roll for a class but your sign-up paperwork would get back to me later than the cut-off date, please contact me. I may have available class space left and could still accomodate you.
